Unfortunately we do not have a back side photo of this gown.  In this photo the gown is tied in the front. It would actually look pretty much the same if tied in the back. There is a slit down the back and depending how large the person is and how tight it is tied it would look closed in the back.  Depending on the procedure the opening could be worn to the front or to the back.
